Fragestellungen:
Es soll in den vier Ländern in den jeweiligen Studienzentren Art und Umfang der physiotherapeutischen und ergotherapeutischen Behandlungsmaßnahmen in der Schlaganfallrehabilitation, die Gesamtorganisation von Schlaganfallrehabilitation, das Outcome und die Nachhaltigkeit (Katamnese) der erzielten Rehabilitationsfortschritte vergleichend evaluiert werden. Mit berücksichtigt wird auch der gesundheitsökonomische Aufwand dafür. Insbesondere soll identifiziert werden, wie der Outcome von der Ausbildung der beteiligten therapeutischen Berufsgruppen, ihren Behandlungseinheiten am Patienten, von den organisatorischen Strukturen innerhalb der Klinik und von äußeren Rahmenbedingungen abhängt.
Methodik:
Es werden demographische und sozioökonomische Daten von je 120 aufgenommenen Patienten und Lebenspartnern erfaßt. Alle Patienten werden bei Aufnahme und Entlassung zwei, sechs und zwölf Monaten nach dem Schlaganfall mit entsprechenden funktionellen Messinstrumenten auf ihre Fähigkeit und Fertigkeiten im ADL-Bereich und im motorisch-koordinativen Bereich untersucht.
Um die Inhalte der Therapieeinheiten zu vergleichen, werden bei randomisiert ausgewählten Patienten je 15 Physiotherapie- und 15 Ergotherapie-Einzelbehandlungseinheiten auf Video aufgenommen und durch ein europäisches Kollektiv von erfahrenen Therapeuten nach einem bestimmten Kriterienkatalog ausgewertet.
Über halbstandardisierte Interviews mit leitenden Ärzten und kaufm. Leitern der jeweiligen Abteilungen werden organisatorische innere und äußere Bedingungen erfasst und abgebildet. Außerdem werden die beteiligten Therapeutengruppen und Pflegekräfte ebenfalls mit Fragebogen zu ihrer Ausbildung, ihren Rahmenbedingungen, ihren aktuellen Tätigkeiten an Patienten und ihrem subjektiven Erleben der organisatorischen Strukturen in der Einrichtung befragt.
Ergebnisse/aktueller Stand:
Die Studie ist vertraglich von der EU bewilligt worden. Patienteneinschluss und Datenaufnahme weitgehend abgeschlossen, Katamnestische Nachuntersuchungen laufen noch bis 12/2004.
Abschlusskongress in Belgien mit Präsentation der Daten für Feb. 2006 von EU vorgesehen. Zusätzliche Datenauswertung der Pat. aus KHA im Rahmen einer eigenen Doktorarbeit vorgesehen.

Community Evaluation of Stroke Rehabilitation in European Comparison (CERISE)
Research questions:
The type and extent of physiotherapeutic and occupational therapy treatment measures in stroke rehabilitation, the overall organization of stroke rehabilitation, the outcome and the sustainability (catamnesis) of the achieved rehabilitation progress will be evaluated comparatively in the four countries in the respective study centers. The health economic cost of this will also be taken into account. In particular, it will be identified how the outcome depends on the vocational training of the therapeutic professional groups involved, their treatment units on the patient, on the organizational structures within the clinic and on external conditions.
Methodology:
Demographic and socioeconomic data of 120 admitted patients and partners each will be collected. All patients will be assessed for their ability and skills in ADL and motor-coordinative domains at admission and discharge two, six and twelve months after stroke using appropriate functional measurement instruments.
In order to compare the contents of the therapy units, 15 physiotherapy and 15 occupational therapy individual treatment units will be videotaped in randomly selected patients and evaluated by a European collective of experienced therapists according to a specific set of criteria.
Organizational internal and external conditions will be recorded and mapped via semi-standardized interviews with senior physicians and commercial managers of the respective departments. In addition, the participating therapist groups and nursing staff are also questioned by means of questionnaires about their vocational training, their general conditions, their current activities on patients and their subjective experience of the organizational structures in the facility.
Results/current status:
The study has been contractually approved by the EU. Patient inclusion and data collection largely completed, catamnestic follow-up still ongoing until 12/2004.
Final congress in Belgium with presentation of data planned by EU for Feb. 2006. Additional data analysis of patients from KHA planned as part of own doctoral thesis.
