Hintergrund:

Eine an der Praxis der medizinischen Rehabilitation häufig geübte Kritik besteht darin, dass ein großer Teil der Rehabilitanden keine optimale Einstellung gegenüber der medizinischen Rehabilitation in den Reha-Einrichtungen besitzt und deshalb nicht in der Lage ist, die auf Mitwirkung aufgebauten Therapiekonzepte in gewünschtem Umfang in Anspruch zu nehmen.

Ziele:

1. Erfassung der motivationalen Bedingungen für eine aktive Mitarbeit während stationärer medizinischer Rehabilitation.
2. Prüfung des Zusammenhangs zwischen Motivation gegenüber der aktivierenden Rehabilitation einerseits und dem objektiven bzw. subjektiven Reha-Erfolg andererseits.
3. Entwicklung eines empirisch fundierten Messinstrumentes zur Erfassung der individuellen Motivation der Antragsteller gegenüber der aktivierenden medizinischen Rehabilitation.
4. Entwicklung eines Interventionsprogramms anhand der empirischen Ergebnisse.

Studiendesign:

Studie an LVA-Versicherten mit chronischen Erkrankungen des Rückens, der Gelenke und des Herzens. Erhoben wurden zum einen die Selbsteinschätzungen der Rehabilitanden u. a. zu Angst (BSI), Depression (BSI), Motivation (HAMORE - selbstentwickelter Fragebogen), Lebensqualität (SF 36) und zum anderen die Einschätzung der behandelnden Ärzte und Therapeuten u. a. zum Reha-Verhalten (Therapiemitarbeit) und Reha-Erfolg.
Messzeitpunkte: t1 = ca. 6 Wochen vor stationärem Reha-Beginn; t2 = Reha-Beginn; t3 = ca. 1 Woche nach Reha-Beginn; t4 = Reha-Ende; t5 = 6 Monate nach Reha-Ende. Bis zum 30.10.2001 wurden zum ersten Messzeitpunkt in sieben Rehakliniken 1.187 Patienten in das Projekt eingeschlossen.

Ergebnisse:

Die Überprüfung der Skalen Spezifische Konsequenzerwartungen, Spezifische Kompetenzerwartungen und Intention erbrachte sehr gute teststatistische Kennwerte (Cronbach Alpha 0,9371 - 0,7911) sowie Korrelationen (0,459 - 0,131) zwischen diesen Modellvariablen, die dem sozial-kognitiven Gesundheitsverhaltensmodell entsprechen. Es zeigte sich auch ein direkter Zusammenhang zwischen der Ausprägung von Motivationsvariablen (zum Beispiel Spezifische Kompetenzerwartungen) und der Ausprägung der Therapiemitarbeit.
